Request Copyright Permission

In requesting permission, please provide the following information:

1. Copyrighted material

· Author name(s)

· Paper title and

· Publication title (i.e., journal, conference, etc.)

· Full citation (i.e., volume, issue, pages, date)

· All, portions, or figures/tables. If not All, please describe.

2. Purpose to reuse

· Prepare a new paper

· Personal archive

· Institutional repository

· Reprint in a book

· Republication in a journal

· Chapter in thesis, dissertation

· Lecture notes or

· Others, please describe.

3. Publication

· Publisher's name

· Publication title

· Volume, issue, date

· Deadline

Our letter in response will grant permission. In most cases, the only requirements will be to give full credit to the original source and to obtain the author's approval (as a courtesy to the author). In cases involving the republication or sale of the material, a fee may also be required.
